Oral surgery is a dental specialty that involves surgical procedures within the oral cavity and jaw. It is required in various situations, such as wisdom tooth removal, difficult tooth extractions, removal of impacted teeth, bone growth (exostosis) surgery, and the removal of oral cysts or small tumors.


Although the term “surgery” may sound concerning, oral surgical procedures in dentistry are typically well planned, safe, minimally invasive, and relatively quick. Who Is Oral Surgery Suitable For?

1. Individuals with impacted or embedded teeth, such as wisdom teeth or teeth that erupt in an abnormal position, remain beneath the gum line, or exert pressure on adjacent teeth. These conditions may cause pain, swelling, or infection and often require surgical removal by a specialist.


2. Individuals who require complex tooth extractions, such as teeth with deep roots or fractured roots at the gum line or root tip fracture in bone , cannot be removed using conventional extraction methods and require surgical techniques to ensure safety and protect adjacent teeth.


3. Individuals with bony growths in the oral cavity, such as exostoses on the palate or the inner cheek, which may interfere with speech or denture placement. Surgical removal can resolve these issues safely.


4. Individuals with oral cysts or tumors. Even if painless or asymptomatic, any abnormal swelling or lump should be evaluated by a dentist to determine the need for surgical removal and proper diagnosis.


5. Individuals preparing for dental implants or dentures, where pre-surgical modification of bone or soft tissue may be necessary to ensure stable, safe, and long-lasting restorative outcomes. To ensure the best possible outcome, each case is meticulously planned through a collaborative approach between our Prosthodontists and Oral & Maxillofacial Surgeons.

The treatments consist of;:

5.1 Bone Contouring (Alveoloplasty/Torus Removal) is the treatment by reshaping irregular bone growth or 'tori' that creates a smooth, uniform foundation for your dentures. This ensures the denture base sits comfortably on the gums, distributing bite pressure evenly to prevent sore spots and irritation.

5.2 Soft Tissue & Muscle Attachment Revision (Vestibuloplasty) is a surgical adjustment of oral muscle attachments that increases the surface area for your denture to rest on. This procedure significantly improves the retention and stability of removable dentures, allowing for a more secure and confident fit.


6. Frenectomy or Frenotomy for Tongue Tie

When is the Best Time for a Frenectomy (Tongue-Tie Surgery)?

For a Frenectomy, "the earlier, the better" is often the rule to ensure optimal development. The ideal age for the procedure depends on the specific symptoms and goals:


1. Infancy (Newborn to 6 Months)

This is the most highly recommended period if the tongue-tie interferes with breastfeeding.

  • The Reason: Infants may have difficulty "latching," leading to poor weight gain, or causing significant pain for the mother during nursing.
  • The Treatment: At this age, the tissue is very thin with minimal blood vessels. A dentist or pediatrician can often perform a quick "frenotomy" without the need for general anesthesia, and the baby can usually nurse immediately after.


2. Toddler Years (2 – 4 Years Old)

If not addressed during infancy, parents often notice the issue as the child begins to speak.

  • The Reason: Symptoms include speech impediments (difficulty pronouncing certain sounds), inability to stick the tongue out, or the tongue appearing "heart-shaped" when extended.
  • The Treatment: Surgery at this stage frees the tongue for better movement. It is often paired with Speech Therapy to help the child learn new tongue positions for clearer speech.


3. School Age (6 Years and Older)

  • The Reason: A restrictive frenum can pull on the gums, leading to gum recession, or create a persistent gap between the front teeth (diastema).


Summary: What is the Best Age?

  • For Feeding Issues: It should be done immediately after birth or within the first few weeks.
  • For Speech Issues: It is best performed before starting school (around age 3-4) to prevent impacts on the child's communication skills and confidence.


Important Note: Not every tongue-tie requires surgery. If the frenum does not obstruct tongue function, treatment may not be necessary. If you are unsure, we recommend scheduling a consultation with our Pediatric Dentist for a professional evaluation.

Post Operative Care After Oral Surgery

After undergoing oral surgery such as wisdom tooth removal, impacted tooth surgery, or oral cyst/tumor surgery the dental team at Siam Dental Clinic will provide clear, case specific instructions to help ensure a smooth, safe recovery and to prevent long-term complications.


Post-operative care recommendations from Siam Dental Clinic:


1. Bite on the gauze continuously for at least 1-2 hours to help control bleeding. If slight bleeding persists, replace the gauze and continue biting gently.


2. Apply a cold compress to the outside of the cheek during the first 72 hours to reduce swelling and inflammation. Apply intermittently for 15–20 minutes at a time.


3. Avoid vigorous rinsing or using a straw, as suction may dislodge the blood clot and cause re-bleeding. Rinse gently and drink directly from a cup instead.


4. Avoid brushing directly over the surgical site initially, but continue to maintain oral hygiene by brushing other areas as usual. Use mouthwash as prescribed by your dentist.


5. Consume soft foods and avoid very hot, hard, or sticky foods. Choose foods at room temperature, avoid excessive chewing, and drink plenty of water to promote proper wound healing.


6.Avoid chewing on the side where the tooth was extracted or surgery was performed. This prevents irritation to the wound and keeps food debris from entering the site, which could lead to swelling or infection.


7.In some cases, a Chlorhexidine antiseptic mouthwash may be prescribed to prevent infection. Please use it strictly as directed by your dentist, typically for

the first 3 to 7 days following the procedure, depending on the complexity of the case


Gently rinse without vigorous swishing to avoid disturbing the surgical site which may irritate the wound and prolong bleeding. Rinse only after meals and before bed time after ensuring that the bleeding stops.

Can I go home immediately after surgery?
Yes. Most oral surgical procedures, such as wisdom tooth removal or minor oral surgery, are performed on an outpatient basis. Patients can return home immediately after treatment without the need for hospitalization.
How many days off work or school are needed?
For multiple wisdom tooth surgeries or more extensive procedures, taking 1–2 days off is recommended. For simple extractions or minor surgery, most patients can resume normal activities the following day.
Is wisdom tooth surgery very painful?
Most patients experience a feeling of pressure or tightness rather than severe pain. At Siam Dental Clinic, adequate local anesthesia is provided to ensure comfort during the procedure. Mild swelling may occur afterward, which can be managed with cold compresses and prescribed medications.
Can I exercise after wisdom tooth extraction?
Generally, it is recommended to avoid strenuous exercise for at least 2–3 days post-surgery. Physical activity increases your heart rate and blood pressure, which can lead to:

  • Persistent Bleeding: It may dislodge the blood clot that is essential for healing, potentially leading to a painful condition called "Dry Socket."
  • Increased Swelling: You may experience more throbbing pain and swelling around the surgical site.
After the first 3 days, if you feel up to it, you may begin light activities like walking or gentle stretching. However, please avoid high-impact workouts or contact sports for at least one week to ensure the wound is fully stable.
How should I care for the surgical site immediately after the procedure?
The most important thing is to bite firmly on the gauze pad for about one hour. Do not spit out blood or saliva, as this can prevent the bleeding from stopping; instead, please swallow normally. Additionally, avoid touching the wound with your fingers or tongue.
When will the pain and swelling subside?
Swelling typically peaks within the first 48-72 hours (days 2–3 after surgery) and will gradually decrease within a week. We recommend applying a cold compress during the first two days to reduce swelling. From the third day onwards, switch to a warm compress to help soothe the area and reduce any bruising.
What can I eat after my surgery or extraction?
For the first 1–3 days, you should stick to soft, easy-to-chew foods that are not too hot, such as porridge, soup, yogurt, or ice cream (the cold can help reduce swelling). Avoid spicy, hot, or crunchy foods with sharp edges, as well as alcoholic beverages.
Why is it forbidden to use a drinking straw?
The suction from using a straw can dislodge the blood clot that protects the wound. If the clot is removed, it can lead to a very painful condition called "Dry Socket," where the underlying bone is exposed, significantly slowing down the healing process.
Can I brush my teeth as usual?
Yes, you can brush your teeth, but be extra cautious especially near the surgical site. We recommend using a soft-bristled toothbrush and avoiding vigorous rinsing for the first 24 hours.
What warning signs indicate I should contact the dentist immediately?
Please contact the clinic immediately if you experience any of the following:
  • Bleeding that does not stop even after biting on gauze as instructed.
  • Severe pain that worsens despite taking prescribed pain medication.
  • High fever or pus discharging from the wound.
  • Signs of an allergic reaction, such as a rash, facial swelling, or difficulty breathing.
When do I need to have my stitches removed?
Typically, the dentist will schedule an appointment to remove stitches about 7–14 days after the surgery. Even if dissolvable stitches were used, we usually recommend a follow-up visit to ensure the wound is healing properly and is free of infection.
